Applying for an online appointment for OFW INFORMATION RECORD?

Are you Ex-Abroad and planning to work again abroad? One of the requirements in working again in abroad is the OFW RECORDS.

Today, the Government has upgraded its system unlike before you have to go to the POEA Office early morning and line up in long lines. Now,  no more long lines, no more long time of waiting because of ONLINE  APPOINTMENT SYSTEM which has been launched by Department of Migrant Workers.

Request by OFW

1. Online Appointment Form
2. ID document with picture (Passport,Driver’s License, PRC Card,NBI Clearance, Company ID, etc.)

Request by Family of the OFW

1. Online Appointment Form
2. Written consent of the OFW for the release of his/her Information/Record
3. Proof or relationship with OFW (Original/Authenticated/Certified True Copy) from Philippine Statistics Authority (PSA)
   • Spouse (Asawa) – marriage contract
   • Children (Anak) – birth certificate of the OFW’s child
   • Parent (Magulang) – birth certificate of the OFW
   • Sibling (Kapatid) – birth certificates of both the sibling and of the OFW
4. ID document with picture (Passport,Driver’s License, PRC Card,NBI Clearance, Company ID, etc.)
5. For Deceased OFW
   • Original copy of Certificate of Death
   • Original / Scanned copy of OFW’s ID
   • Original copy of CENOMAR / Advisory of Marriages

Register / Make appointment

Request by other parties

1. Letter of request address to the Chief of Central Records Division
2. Others
   • Persons authorized by the OFW – Special Power of Attorney (SPA)
   • Government/Private Agencies – Proof of Authority of representative to receive the record
   • Parties with case filed in court – Court Order to the POEA
3. ID document with picture (Passport,Driver’s License, PRC Card,NBI Clearance, Company ID, etc.)
4. Proof of any circumstances under Section 13 of the Privacy Data Act of 2012
5. Other legal documents as may be needed
