📋 Documentary Requirements

Applicants must prepare the following documents upon application:

Basic Requirements:

• Valid Passport (at least 1 year validity)

• NBI Clearance (Multipurpose)

• PSA Birth Certificate

• PSA Marriage Contract (if applicable)

• Educational Documents:

  • Form 137 (if High School Graduate)

  • Diploma & Transcript of Records (if College Graduate)

• UMID (Unified Multi-Purpose ID)

• Voter’s ID or Voter’s Certificate (or Certificate of Non-Availability of Record)

• 3 Valid Government IDs (TIN and other IDs)

• SSS Printout (Member’s Data, Contribution List, and Employment History)

• Printed PEOS Certificate

• Printed E-Registration Certificate

Additional Requirements for Ex-Taiwan Applicants:

• Old Passport (with entry and exit visa stamps)

• ARC / Health Insurance Card

• If Old Passport is Lost:

  • Certificate of Entry and Exit from Bureau of Immigration

  • Notarized Affidavit of Loss
