b. Voter’s ID or Voter’s Certificate with full details, biometrics, and dry seal
c. National ID or printed copy from eGovPH app
d. SSS Member’s Details, Contribution Records, and Coverage History (new landscape format)
e. PEOS Certificate
f. E-Registration Certificate
g. PSA Birth Certificate with QR Code

Ensure your PSA Birth Certificate includes your middle name and full names of both parents.
