Requirements:

Basic Documents:

• Diploma or Certificate of Graduation, Form 137 (for High School Graduates)

• Diploma or Certificate of Graduation & Transcript of Records (for College Graduates)

• UMID (Unified Multi-purpose ID)

• Voter’s ID or Voter’s Certificate from COMELEC, Certificate of Non-availability of Registered Record

• 3 pcs valid IDs (TIN ID & other government IDs)

• SSS Printout (Member’s Data, Contribution List, and Employment History)

• Printed PEOS Certificate (PEOS)

• Printed E-Registration Certificate (POEA E-Registration)

Additional Requirements for Ex-Taiwan Workers:

• Old Passport (with entry & exit visa)

• ARC / Health Insurance Card

• Certificate of Entry and Exit from Bureau of Immigration

• Notarized Affidavit of Loss (if old passport is lost)
