- PEOS Certificate
- Colored copy of passport
- If no passport yet, a copy of the DFA appointment slip and valid government ID may be submitted
- Diploma and TOR or Form 137
- PSA Birth Certificate
- PSA Marriage Certificate — for married applicants
- NBI Clearance
- Two (2) 2×2 ID pictures with white background
