  • Printed PEOS Certificate

  • Printed POEA E-Registration Certificate

  • Old Passport (with entry & exit visa)

  • ARC / Health Insurance Card

  • Certificate of Entry and Exit from Bureau of Immigration

  • Notarized Affidavit of Loss (if old passport is lost)
